The COSY System is a modern, modular posterior occipito-cervico-thoracic (OCT) fixation platform designed to support constructs from the occiput to the upper thoracic spine within a single system – including the cervicothoracic junction through dedicated transition rods.
A key strength of COSY is its freedom of anchor selection. Polyaxial, cervical pedicle, smooth-shank, and angled screws (Ø 3.5–4.5 mm) allow surgeons to combine lateral mass, pedicle, C2 pars, and translaminar trajectories within a single construct, adapting fixation to the individual anatomy and surgical requirements. Low-profile, cannulated screw heads with up to 70° of angulation support K-wire-verified trajectory control and facilitate navigated and tissue-preserving techniques. For complex anatomy and intraoperative flexibility, the system is complemented by occipital plates and rods, cross and offset connectors, as well as atlas hook options, providing a broad range of construct possibilities within one modular platform.
